You will need a referral from you General Practitioner to access specialist medical and surgical services. This is not a requirement for allied health consultations.

Should you have a referral from your medical practitioner, feel free to upload it on the contact us page.
Alternatively, you may fax (02 8080 8115), email (ad***@co*********************.au) or drop off your referral to one of our clinics.
If your referral is urgent please indicate this clearly at the time of submitting the referral.

If you would like to use our Patient Portal, please do the following: 1. Login with the details provided by email when we created your account. 2. Please take a few moments to check that your personal information within the Patient Portal account is complete and current. 3. Use the Patient Portal to book your appointment. Please remember that you must have a valid referral from your GP or a medical specialist to book an appointment with us. Please also make sure that your medical history is current. Please bring a completed patient history form to your appointment.
